Copyright notice
- Identify the copyrighted work and the material claimed to infringe.
- Provide the location of the material and your contact information.
- State your good-faith belief that the use is unauthorized.
- State under penalty of perjury that the notice is accurate and you are authorized to act.
- Provide a physical or electronic signature.
Counter-notice
A person whose material was removed may submit a legally sufficient counter-notice. We may provide notices to the affected member and act as required by law.
Designated agent
The designated-agent appointment and public contact details require completion by qualified counsel before this draft can be relied upon for DMCA safe-harbor purposes.
